Had an accident? Report it here. Send what you have — a licensed claims representative takes it from there, usually within one business day.
At the scene right now?
If anyone is injured, call 911 first. Then call us — we'll walk you through it and open the claim over the phone.
What to have ready
The form asks for all of this. Gathering it first turns the whole thing into about ten minutes.
Date, time and exact location
Street, city and state — plus the highway and mile marker if it happened on the road.
The driver's signed statement
Written and signed by the driver who was involved. A photo of a handwritten page is fine. English is preferred.
Your driver, and your equipment
Driver name and phone, plus the truck and trailer with their VIN numbers.
The other vehicle, if there was one
Driver name and phone, vehicle and plate, and their insurance company.
The police report
If law enforcement responded: the department name, the case number, and a copy of the report.
The load, if the truck was carrying cargo
Whether the cargo was damaged, plus the Bill of Lading and the Rate Confirmation.
Photos and video
Both vehicles, the scene, the road, and every area of damage. More is always better.
Missing something? Report it anyway. We would rather open the claim today with half the details than wait a week for a complete file — you can send the rest by email afterwards.
